Welcome to the Dan Peterson School Nurse’s web page. We strive to provide the best nursing care for all of the students here at Dan Peterson. Along with day to day care of our students, a big part of our job is ensuring all of our students are in compliance with the State of Utah immunization requirements. If a student needs a medication given at school an “Authorization of Medication” form needs to be filled out and signed by a doctor. This form and the medication will need to be brought to the school by a parent or guardian. Medication must be checked in with a nurse and cannot be sent on the bus or in a backpack. All medication forms must be updated at the beginning of each school year, or if any changes in medications are made. We are here to help, so please do not hesitate to contact us at 801-610-8189.
